Who we serve
White Glove DME handles enrollment, accreditation readiness and state licensing for DMEPOS suppliers: medical supply companies, pharmacies, O&P practices and mail-order suppliers. Your authorized official signs every certification, and you pay every government, accreditation and bond fee directly. We do not sell equipment or surety bonds. Commercial payer networks are handled by our White Glove Credentialing sister team.

DMEPOS Readiness Review
$149 one-time
Before you spend anything: which states you ship to require a license, whether your accreditation covers every product you sell, how your location measures against the 30 supplier standards, your bond and insurance, and where competitive bidding or a moratorium could touch you. Credited in full toward your first package when you go ahead.
Medicare Enrollment
Medicare DMEPOS Enrollment Package
$1,295 first location
The CMS-855S, the supplier standards evidence, bond and insurance documents, and site visit preparation. Once your authorized official approves us as a PECOS surrogate, we enter and submit the application; your official reviews and signs it. We follow it to a decision.
Additional Location Enrollment
$595 per location
Each location enrolls separately, with its own bond rider and accreditation notice. We prepare all three for each new site.
Medicare Revalidation
$595 every three years
Every supplier revalidates every three years, with a new fee and a bond. We re-verify your licenses and accreditation, prepare the revalidation and track it.
Change of Information
$249 per change
Changes must be reported within 30 days: a new address, officials, owners, products or accrediting organization. We prepare the change and update every record that has to match.
Ownership Change Review
$995 per transaction
A majority ownership change within 36 months of enrollment means a new enrollment and new accreditation. We review the deal against the rule and plan the enrollment work before you close.
Accreditation
Accreditation Readiness
$2,495 one-time
A gap analysis against the CMS quality standards, a policy and procedure manual fitted to your products, staff training records and a mock unannounced survey. You apply to the accrediting organization and are surveyed; we prepare you for it.
Annual Survey Readiness
$1,200 per year
From 2026 suppliers are resurveyed every 12 months. A yearly mock survey, log and file audits, and policy updates keep you ready for an unannounced visit.
State Licensing
State DME License
$699 per state
The state's license, permit or registration for your products, prepared for your owner or officer to sign. We send it where the state takes it by mail or email and track it to a decision.
Complex State DME License
$899 per state
California, Florida, Georgia and Alabama add more: an exemptee, an accreditation deadline, a designated representative or an in-state location. We prepare the extra pieces too.
Remote Delivery Licensing Package, 10 States
$4,995 for ten states
For mail-order suppliers: ten state license files prepared together, with the licenses loaded into PECOS. Built for Round 2028 bidders, who need every required license in PECOS by the close of the bid window.
Nationwide Licensing Package
$9,995
Every state that requires a license for your products, prepared and tracked, with one renewal calendar for all of them.
Medicaid DME Enrollment
$499 per state
Medicaid enrollment on top of Medicare, with any state bond, site visit or Medicare-first rule planned in.
Ongoing
License Renewal Tracking
$299 per license, per year
State licenses renew on their own dates, from annual to every three years. We track each one, prepare the renewal and confirm it went through.
Important Fee Notice
(Prices above do not include the following)
The prices listed above are our service fees only. Costs paid directly to third parties are set by others and change from time to time, so we confirm them at intake:
- The CMS application fee: $750 for applications in 2026, paid to CMS with each enrollment, revalidation and new location.
- Accreditation fees: set by the accrediting organization you choose, now for a survey every year.
- Surety bond premiums: paid to your own surety agent; the premium depends on your credit.
- State license fees: set by each state, often per location and renewed every year.
- Fingerprinting: for every owner of 5% or more when CMS screens a new supplier as high risk.
